The Canadian Cooperative Wildlife Health Centre is looking into the health of Garden Birds in the Atlantic Provinces. Garden birds are those wild birds frequently found around homes, in public and private gardens or yards, particularly those that visit bird feeders and water baths.
By filling out this questionnaire you will help us get information that we can use to evaluate the health status of garden birds across the Atlantic Provinces. If you have a bird feeder or a water bath and would like to participate, please answer the following questions. It will take you approximately 10 minutes to complete the questionnaire.

Your participation in this study is completely voluntary. If you feel uncomfortable answering any questions, you can skip it; you can also withdraw from the survey at any point. Your survey responses will be strictly confidential and data from this research will be coded and will remain confidential.
